Bile acid malabsorption

Bile acid malabsorption (BAM) is one of the causes of diarrhoea and irritable bowel syndrome. Bile acids are important for the absorption of dietary fats and fat soluble substances. Bile acids are synthesized in the liver, stored in the gallbladder between meals and excreted into the duodenum in response to ingestion. Normally more than 90% of bile is reabsorbed in the small bowel, as part of the enterohepatic circulation. When this does not occur as in primary (idiopathic) or secondary (following cholecystectomy or right hemicolectomy) bile acid malabsorption, excessive bile acids enter the colon, resulting in diarrhoea.
